Benefits of Use

• Locally advanced or metastatic non-small cell lung cancer (NSCLC), often in combination with cisplatin or as a maintenance therapy.
• Malignant pleural mesothelioma, in combination with cisplatin, for patients who are unresectable or otherwise not candidates for curative surgery.

How to Use

1. Reconstitution: The lyophilized powder in the vial must be reconstituted with sterile water for injection, following the specific instructions provided in the package insert, to form a clear solution.
2. Dilution: The reconstituted solution is then further diluted with 0.9% sodium chloride injection to a final concentration suitable for intravenous infusion.
3. Administration: The diluted solution is administered as an intravenous infusion over a period of 10 minutes. The exact dose and infusion schedule will be determined by the treating physician based on the patient's body surface area, type of cancer, and overall health status.
4. Pre-medication: Patients typically receive pre-medication with corticosteroids, folic acid, and vitamin B12 to reduce the incidence and severity of treatment-related toxicities.
5. Monitoring: Regular monitoring of blood counts, kidney function, and other relevant parameters is crucial throughout the treatment course.

Caution & Warnings

• Myelosuppression: This drug can cause severe bone marrow suppression, leading to neutropenia, thrombocytopenia, and anemia. Close monitoring of complete blood counts is essential.
• Renal Impairment: Pemetrexed is primarily excreted by the kidneys. Patients with impaired renal function require dose adjustments and careful monitoring to avoid increased toxicity.
• Dehydration: Adequate hydration is important before, during, and after pemetrexed administration.
• Drug Interactions: Concomitant use with non-steroidal anti-inflammatory drugs (NSAIDs) should be avoided or carefully monitored, especially in patients with mild to moderate renal insufficiency.
• Pregnancy and Lactation: Pemetrexed can cause fetal harm and is contraindicated during pregnancy. Women of childbearing potential should use effective contraception. It is not known whether Pemetrexed is excreted in human milk, and breastfeeding should be discontinued during treatment.
• Extravasation: Care should be taken during administration to avoid extravasation, which can cause tissue damage.
• Immunosuppression: Due to its immunosuppressive effects, patients may be at increased risk of infection.
