Caution & Warnings
- Cardiotoxicity: Epirubicin can cause dose-dependent cardiac toxicity. Regular monitoring of cardiac function, including ECG and ECHO, is essential before and during treatment.
- Myelosuppression: Severe bone marrow depression (decreased white blood cells, red blood cells, and platelets) is a common and dose-limiting toxicity. Complete blood counts must be monitored frequently to manage this risk.
- Extravasation: Accidental leakage outside the vein can cause severe local tissue necrosis. Administer cautiously and monitor the injection site closely for any signs of extravasation.
- Secondary Malignancies: There is an increased risk of developing secondary acute myeloid leukemia (AML) and myelodysplastic syndrome (MDS) following epirubicin therapy.
- Gastrointestinal Effects: Nausea, vomiting, mucositis (inflammation of mucous membranes), and diarrhea are common side effects. Prophylactic antiemetics may be required.
- Alopecia: Reversible hair loss is expected.
